If an engine was faulty or badly vented it could cause build up of CO in the cockpit leading to dizziness or even death.

• Piston powered aircraft produce high concentrations of carbon monoxide entering the cabin during flight

The most common way of large amounts of CO to find its way into the cabin of a single-engine piston aircraft is through the cabin heat system. When cabin heat is being used, any cracks or holes in the muffler can allow CO rich exhaust gas to contaminate the cabin.
