Gentamycin is an aminoglycoside drug.Its mechanism of action is to act on ribosomes in bacteria,inhibit bacterial protein synthesis,and destroy the integrity of bacterial cell membranes.
Gentamicin can first pass through the extracellular membrane porin by passive diffusion,then enter the cell through the cell membrane via the transport system,and irreversibly binds to the isolated 30S subunit of the ribosome,resulting in destruction of the A-site, and thus:
1. To prevent the correct localization tRNAs in the A-position,especially to interfere with the
binding of methionyl-tRNA, thereby interfering with the assembly of functional ribosomes and inhibiting the formation of the 70S initiating complex.
2. Induced mismatch between tRNA and mRNA crypto-triad,causing the 30S subunit of the intact
ribosome to misread the genetic code,causing erroneous amino acid insertion into the protein
structure,resulting in abnormal,non-functional protein synthesis.
3. Impeding the termination factor from binding to the A position, so that the synthesized peptide chain
cannot be released and prevent the 70S intact ribosomes from dissociating.
4. Blocking the depolymerization and assembly of polyribosomes,causing ribosomal depletion in bacteria.
Gentamicin against Pseudomonas aeruginosa,Proteus (positive and negative),Escherichia coli,
Klebsiella, Enterobacter, Serratia, Shigella, Citrobacter Neisseria, Staphylococcus aureus (excluding
methicillin-resistant strains) has strong antibacterial activity.
Gentamicin is not effective against streptococci (including Streptococcus pyogenes,Streptococcus
pneumoniae,Streptococcus faecalis, etc.),anaerobic bacteria (Bacteroides),Mycobacterium
tuberculosis,Rickettsia,viruses,and fungi.
1. Applicable to the treatment of sensitive bacteria caused by neonatal sepsis, sepsis.
2. Applicable to the treatment of respiratory tract infections, peritonitis, biliary tract infections caused
by sensitive bacteria.
3. Applicable to the treatment of urogenital infections caused by susceptible strains (other than initial
treatment of simple urinary tract infections).
4. Applicable to the treatment of skin and soft tissue infections caused by susceptible bacteria,burns and
other infections.
5. Oral can be used for bowel infection or preparation before colon surgery
6. Intramuscular injection of clindamycin or metronidazole in combination can be used to reduce the
incidence of infection after colon surgery.
7. Intrathecal injection can be used as adjuvant therapy for severe central nervous system infection
(acute purulent meningitis,ventriculitis)caused by Pseudomonas aeruginosa or Staphylococcus
aureus.
8. Gentamicin can also be used to treat listeriosis.
9. Gentamicin eye drops apply to coagulase-negative and positive staphylococci,Pseudomonas
aeruginosa and E. coli, K. pneumoniae and other gram-negative bacilli and gonococcal conjunctivitis,
keratitis,tears Bladder inflammation,blepharitis,meibomian gland inflammation,etc.
