Glimepiride is a third-generation sulfonylurea long-acting antidiabetic drug whose main mechanism of hypoglycemic effect is to stimulate insul in secretion from pancreatic β-cells and partially improve the sensitivity of surrounding tissues to insul in. It binds to and dissociates from insul in receptors faster than glibenclami de and is less likely to cause severe hypoglycemia. The mechanism of action is through binding to the sulfonylurea receptor (a protein with a relative molecular mass of 6.5×104) on the surface of pancreatic β-cells, which is coupled to ATP-sensitive K+ (KATP) channels, prompting the closure of KATP channels, causing depolarization of cell membranes, opening voltage-dependent calcium channels, inducing insul in release by Ga2+ inward flow, and inhibiting hepatic glucose synthesis. Recent studies have shown that different sulfonylurea coal drugs have different affinities for KATP. The order is: euglycemia = glimepiride > toluenosulfonylurea > chlorosulfonylurea. In addition, glimepiride can also increase cardiac glucose uptake through a non-insul in-dependent pathway, which may be due to the increased action of two proteins, glucose transport factor 1,4, expression. Since the effect of glimepiride on cardiovascular KATP channels is weaker than that of glibenclami de, gliclazide and glipizide. Therefore, cardiovascular adverse effects are also rare.
